Reading comments I saw that the formatting helps you understand. With reality being aligned left, Frank's hallucinations aligned right and memories in the middle. Once I got that it was easier to work out what was happening in the moments I didn't understand at first. Because like you, I got lost at times, but I think that added to the whole mixed-up trippy feel of the story.
As Mikey goes cold turkey on his meds and Frank gets more fucked up in the head and there were parts of it that I read with tears flowing and my hand pressed against my mouth because, ouch.
The two of them against the world and it really was fucked up, because they weren't meant to be fleeing for their lives and just barely surviving and man, everything after Mikey being beaten up. I echo your wow.
